2023.05.31 02:16 HappyCrappie SO experienced first manic episode since 2013 (Bipolar 1). They've been hospitalized 10 days so far and still haven't heard from a psychiatrist, so I came here with questions.
Just some background details to explain the questions. Skip ahead if you want: SO diagnosed with Bipolar 1 in 2011 at age 21. Proceeded to have 3 manic episodes requiring hospitalization from 2011-2013. Following the 3rd hospitalization, she was placed on daily lamotrigine, Latuda, wellbutrin, PRN Xanax, and after like 6 months of euthymia, she was prescribed 40 mg adderal XR for ADHD. After about 2 years, she developed a daily drinking habit. Fast forward to 2023, she stopped drinking cold turkey in March (was never severe enough to cause withdraw) and started taking CBD supplements to help her sleep (she was used to just falling asleep after drinking). Due to the adderal shortage, she was prescribed Vyvanse at the end of March. She was experiencing work stress and heard her abusive, controlling high school boyfriend had died of an OD back in October 2022. For reasons i can't comprehend, she struggled with his death (he is the reason she also had PTSD.)
Weirdly, the alcohol cessation corresponded with the work stress. And while I was concerned about the work stress, she had dealt with similar issues in past (bad managers) and got by fine. What concerned me the most at the time was the damn Vyvanse. Even though she thought it made her more productive, she also kind of became an irritable asshole. 1 week later, she was hospitalized.
Anyway, on to my questions:
- Could the change from Adderal XR to Vyvanse been a factor in the irritability and mania development or just a coincidence? I know mania doesn't occur at the flip of a switch, but the personality change with the Vyvanse was significant.
- Is there any reason to believe that her quitting drinking could have contributed to her mania development? At the very least it would have affected how her body processes her medications, right?
- Previously, she had been trialed on Lithum, Geodon, and seroquel, which either didn't work or had bad side effects. Is there like a algorithm that helps you decide which meds to try next, or is it more patient specific?
- On the topic of meds, I'm reading that Latuda and Lamictal are prescribed primarily for Bipolar Depression. I don't think I've ever seen her experience the depressive side so I guess the meds are working for that, but why wouldn't she have been on something to prevent mania too?
- When someone is admitted inpatient for BPD mania or depression, do you keep them on what they were on or start fresh with the assumption that the current medications or dosing aren't working?
- Update: The social worker called will I was typing this. Previously, she was prescribed 80mg Latuda and 200mg Lamictal(I think). Now she's off Latuda, on 50 mg Lamictal, and 2mg risperidone. Can you give me an educated guess as to why they made these med changes? The social worker wasnt sure.( I see risperidone is for mania so that makes sense)
I guess that's all for now. If this gets posted and not removed, thanks for a reply to any of the questions. Maybe one day I'll actually get to talk to the psychiatrist taking care of my wife.

"Perth-based regenerative medicine company Orthocell has launched a new bio-manufacturing facility in what managing director Paul Anderson says represents the successful translation of quality science into a world-class product.
Orthocell’s expansion follows a $23.1 million global exclusive marketing and distribution deal with one of the world’s largest dental implant companies, BioHorizons, for its Striate+ product.
The new facility in Murdoch has the capacity to produce up to 100,000 units of Striate+ a year, up from 10,000 units.
“The fact that we’ve been able to develop a product, to have that product regulatory-approved across European, American and Australian jurisdictions ... is a really major milestone for not just our company but for the Australian manufacturing environment,” Mr Anderson said.
“This new bio-manufacturing facility is going to carry the bulk of the load of that new manufacturing requirements to meet our partner’s needs.”
The ASX-listed company received the first purchase orders from BioHorizons for Striate+, a resorbable collagen membrane used for dental-guided bone and tissue regeneration procedures, in September last year.
The Striate+ product was launched in the US market in November.
“One of the big challenges that we have as a company based in Australia is access to global markets,” Mr Anderson said.
“Australia is a great environment for the development of these technologies and ... we’re passionate about maintaining the manufacturing capability, the intellectual property.”

I’ve always been an aquaphor girl for chapped lips but the past 2 years or so, my lips had suddenly gotten BAD—as in, every few days my lips would go up in flakes as if I had done some sort of wacky chemical peel BAD, couldn’t go to a 30 minute meeting in a conference room without taking a tube with me BAD. It was horrible and so embarrassing, especially given my job is in senior corporate client management and very high-visibility.
Derm thought it might be lip lickers’ dermatitis so I doubled down on the Aquapholanolin to make sure I didn’t get dry enough to want to lick them. Nothing.
During this time I heard about how some people with perioral dermatitis benefit from SLS free toothpaste. But I thought, hey, I’ve been using Crest literally my whole life with no issue, how could this matter?
Then recently I went on a trip where I had to use someone else’s Sensodyne for a few days. Within DAYS it was significantly better. Holy crap.
If you have been thinking about this, or suffer from lips that hate you, don’t be like me and suffer for years for no reason. Get the damn SLS-free toothpaste already!!
Disclaimer: not all Sensodyne toothpastes are SLS-free, I believe, but many are. I went with Sensodyne Extra Whitening.

The People’s Republic of China (PRC) is the world’s emerging superpower. However, it is also a country facing a growing number of economical, social, and ecological issues.
China’s GDP exceeds $11 trillion dollars, making it the second largest in the world after the US. Despite this, its rate of economic growth has sharply dropped to 6% in 2016 from 9% in 2013. The PRC has a highly centralized and government-managed economy, along with strictly regulated political life. Notwithstanding, some Chinese provinces are allowed to pursue independent economic policies.
China officially has a multi-party political system. However, in practice, there is only one party that holds significant power — the Communist Party of China (CPC), which was founded on July 1, 1921. The CPC’s ideology is based on the Marxist-Leninist doctrine introduced by Karl Marx (a Prussian economist) and Friedrich Engels (a German philosopher and businessman) in the 1880s. This doctrine was later supplemented by the theory of Socialism by Vladimir Ulyanov-Lenin (a Russian politician and revolutionary).
The doctrine postulates that society is composed of two major economic classes — the working people (exploited) and the bourgeoisie (exploiters). According to this theory, a global economic crisis will lead to a revolution, with the working class taking control of the world and the bourgeoisie being eliminated. Ultimately, capitalism will cease to exist, and communism (where goods are freely available and nobody works) will prevail. Mao Zedong, the founder of the CPC, adapted this theory to the Chinese context by including Chinese farmers in the definition of the working class.
The PRC government plays a central role in both the political and economic aspects of China. The CPC governs China through an administrative pyramid, with the Central Committee (Politburo) and the National Congress at the top.
The National Congress is comprised of over 2000 delegates who are elected mainly from local CPC committee members. This assembly convenes for a two-week session in Beijing once a year. The 22 Chinese provinces are managed by regional governors appointed by the Central Committee, and their primary objective is to fulfill the CPC’s goal of achieving GDP growth. The government operates based on five-year plans.
The list of other political parties in China includes:
Revolutionary Committee of the Kuomintang (53,000 members, representing Taiwan residents in China); China Democratic League (130,000 members, mainly composed of the middle class); China Democratic National Construction Association (69,000 members, consisting of entrepreneurs); China Association for Promoting Democracy (64,000 members, primarily intellectuals); Chinese Peasants’ and Workers’ Democratic Party (65,000 members, representing government employees); Zhigongdang of China (15,000 members, representing overseas Chinese); Jiusan Society (68,000 members, comprising individual professionals); Taiwan Democratic Self-Government League (1,600 members, including prominent Chinese celebrities). All of these parties are aligned with and support the CPC in its major initiatives and policies. Opposition to the CPC is not tolerated.

In March 2023, China’s surveyed urban unemployment rate decreased to 5.3%, the lowest in seven months, from February’s 5.6%. The unemployment rate in 31 large cities and towns also declined to 5.5% from 5.7%.
Employees’ average weekly working hours across China increased to 48.7 in March from 47.9 in February. In the first quarter of 2023, the unemployment rate slightly declined to 5.5% from 5.6% in Q4 2022. The government has set a target of around 5.5% for the year, with the creation of approximately 12 million new urban jobs. China has also set a 2023 GDP growth target of about 5%.

China’s economy grew by 2.2% (SA) in Q1–2023, the third consecutive quarter of expansion following the removal of travel restrictions in Dec-2022 and a three-year crackdown on tech firms and property. However, the uneven recovery showed that while consumption, services, and infrastructure spending picked up, slowing inflation and rising bank savings led to doubts about demand.
In Mar-2023, the central bank cut lenders’ reserve requirements for the first time in 2023 and Beijing promised more fiscal stimulus.
Inflation
In April of 2023, China’s inflation rate declined to 0.1% from the previous month’s 0.7%, which was lower than anticipated. The decrease in prices for both food and non-food items was due to an unstable economic recovery after the enclosure policy was lifted. Food prices fell notably due to lower prices of pork and fresh vegetables, while non-food prices fell due to lower prices for transportation and housing. Inflation for health remained steady, while education costs increased.

Country’s exports rose unexpectedly by 14.8% YoY to a high of USD 315.59B in March 2023, rebounding sharply from a 6.8% drop in January-February combined and beating market consensus of a 7% fall. It was the first advance in shipments since September 2022 as Beijing boosts trade with developed countries and emerging economies. Steel products (53.2%) and refined products (35.1%) were the largest contributors. Exports to China’s largest partner, ASEAN, rose 35.43%, while those to the EU (3.38%) and Russia (136.43%) also increased. Conversely, exports fell to Japan (-4.8%), Taiwan (-27.6%), and the US (-7.68%), while they expanded to Australia (23.7%) and South Korea (11.3%).

The Chinese reforms were initiated by Deng Xiaoping (1978–1992) in the late 1970s. These reforms aimed to open up China to foreign investment, encourage private enterprise, and modernize various sectors of the economy. As part of these reforms, the number of directly controlled industries was drastically reduced. Additionally, the number of price-controlled goods decreased from approximately 300 to around 20.
With that said, the government still exercises strict regulatory oversight and establishes guidelines and policies that private businesses must adhere to. However, when Xi Jinping (2013-present) assumed power, there was a gradual shift towards increased direct control over the economy and the establishment of a more centralized system. This change was accompanied by the active implementation of mass-surveillance technologies.
That happened synchronously with the end of the world’s latest 80-year-long generational cycle, which began in the 1940s and 1950s. This cycle was characterized by massive political decentralization, resulting in the emergence of several dozen new states between 1940 and 2000. After the 2007–2008 debts debacle, this cycle of economic expansion, driven by the exploitation of readily available resources, came to a close. However, it was artificially extended for the next 15 years through the easing of monetary policies pursued simultaneously by central banks worldwide.
That led to an unprecedented growth of private businesses worldwide. It was accompanied by increased prosperity and a rising level of education across all segments of the population. Small and medium-sized entrepreneurs, particularly in the high-tech industry, began to assume leading positions in the economic landscape. However, this economic progress was not accompanied by significant political reforms.
The old class of hereditary, mostly populist politicians, who often lacked education, managed to stay in power throughout that period, largely due to the outdated electoral system based on indirect political representation. However, when blockchain technologies were utilized to establish algorithmic consensus and enable effective direct governance, this new system faced resistance from entrenched political clans in all countries, resulting in its suppression.
China is currently at the forefront of this trend, with its political class focused on leveraging high-tech advancements to achieve both economic efficiency and comprehensive political control. However, there are two significant obstacles that China faces along this path.
Firstly, China is confronted with a shrinking population. In recent years, the country has undergone a substantial demographic shift characterized by an aging population and a decline in the working-age population. This is partially attributed to the one-child policy that was enforced from 1979 to 2016, resulting in a diminished labor force and a growing proportion of elderly individuals.
Secondly, there is a culmination of the resource-exploration and expansionist phase of global economic growth, accompanied by escalating political and military tensions worldwide. This situation is likely to result in a reduction of China’s import markets and an increased dependence on a less-competitive domestic market. Consequently, this could potentially lead to a scenario of stagflation, characterized by stagnant economic growth coupled with high inflationary pressures.
Faced with these fundamental challenges, it is highly probable that China will resort to aggressive and militaristic policies in an attempt to expand its territory directly or enforce its economic dominance in the Asian region through alternative means.
China is expected to continue on its trajectory towards increased global dominance through local conflicts and enhanced technological control over the economy and population over the next 15–20 years. However, this trend could be altered by a new wave of decentralization, which would require a significant deviation from current policies.
Such a shift may occur when not only the current generation of older politicians, but also the subsequent one (which is likely to further reinforce the existing trend), is replaced by “enlightened” technocrats who advocate for a return to decentralized approaches in both politics and economics.

I'm a senior in high school, graduating in a few weeks, and I'm unsure whether I should attend the ceremony. Although my parents are looking forward to it, I'm completely dreading it. My high school experience hasn’t been too great. I attend a large private school on a scholarship and while I’m incredibly fortunate to have went, it has been incredibly stressful and competitive. Throughout every single year, I have constantly cheated (and recently got caught), and my attendance was lackluster to the point where I was almost kicked out. Despite that, I’ve managed to graduate with a good enough gpa. But in all honesty, I don't believe I deserve to graduate, let alone be celebrated. It doesn't seem worthwhile to attend a ceremony for something I didn't truly earn and pretend to be happy for the sake of my friends/family. My parents know about some of the incidents but still want to attend the ceremony and take pictures, but I feel like those will be the longest 3 hours of my life. What should I do?

I’ve been looking for a job in my field for months since I quit my last role.
Now, I was recently offered a position (after three rounds of interviews even with the most senior manager at the company) and the salary they offered me was decent. It’s around $15k more than my last position, and honestly i’ve been looking for a job for so long (since everything around me is falling apart) that I accepted the offer without batting an eye.
Now, my partner is upset/confused by my actions. He claims I should always negotiate, especially for salaried positions, as they typically have some wiggle room and I could make even more. Also, Glassdoor shows that people in my role typically make about $10k more than I was offered. I told him that the site isn’t always realistic but I digress.
While I would normally agree with his reasoning, I didn’t think negotiating was a wise idea since I had no leverage. The company knew I was unemployed (since I had actually left one of their subsidiary companies) and honestly them even extending an offer felt extraordinary to me. Also, to be frank, it’s hard to get the kind of job I was offered with my level of experience at this point.
What do you all think? Was I unwise in my decision? Would I be able to renegotiate a salary increase after a few months if I find that my coworkers are making more?

I've been at my company working as a front-end web developer for about two years now (making $90k and working remotely,) and I've been considering getting a new job, as the level of dysfunction in my company is bothering me. However, I want a reality check to figure out if this is just a case of the grass being greener. Here's how things are going right now:
Management-wise:
They have a habit of hiring new college hires in bulk (that's how I was hired) while also not compensating their senior developers properly, leading to mid to senior level developers leaving for better jobs, hampering productivity as there are too many heads without enough guidance to execute things.
A while back, the company laid off several hundred people (I'm not sure how many were developers) who received poor performance reviews. I talked to one of my coworkers who was laid off, and she said that shortly before she was fired, her manager told her she had time to recover from this review. Behavior like this is why members of my team have an appropriate level of distrust toward the management, and we regard their announcements with jaded skepticism.
Tech-wise:
We have been trying to convert our product from AngularJS to Angular. There was actually an attempt to do this a few years back (before I joined the company) but the effort was scrapped, so we basically waited until the eleventh hour to get it done, and much of our product still runs on (unsupported) AngularJS (we run a hybrid app.)
Our architecture is in shambles. We have one lead developer that knows all of the quirks and bandaids of our product (in one case we check if a user is logged in by throwing an exception, lol,) and one who was able to get approval to create an entirely new copy of our application from the ground up using proper architectural practices. As of a week ago we now run a hybrid app of her new application as well as the old (hybrid) app.
Not related to code architecture, but we've also been instructed not to put markup in our CMS database while also being told that it's already been done too much to try and fix it now.
Our environments are constantly breaking. It feels like most weeks at least one person's environment breaks, and only slightly less often does everyone's environment break all at once. This is usually resolved within a day or two, but sometimes can take longer. This is because even when we run our front and back end locally, we still depend on several remote services.
Our product is spaced out across multiple (interdependent) repositories and teams (and by multiple repositories, I mean literally dozens if not hundreds of which there are probably a dozen that are currently in use); often one team will make a mistake that takes out the environment for several teams, but since we only have access to our own repositories, we can't even help to diagnose and fix other teams' issues when they're resolved.
We have one lead developer that seems to be the only one that knows how to fix these environment problems. Whenever someone else helps me resolve my environment, they almost never know why it works but simply say that our lead developer explained it to them.
Task managers/Scrum leads have no idea what's going on. Often I'll make a change that's as simple as adding a CSS class to an HTML attribute (literally a one line change) but because of poor communication between the team that wanted the styling changed, the task manager, and the developer working on the task, it can easily take 3+ weeks for this change to be merged, during which there may be meetings scheduled where the expected behaviostyling is discussed, or the change may be merged to the wrong branch or repository.
It's nearly impossible to debug anything because the JavaScript console on the frontend and output in the backend normally return a ton of errors and warnings; when errors pop up that actually affect the environment, they're buried in the noise. It's likely that many of these issues are coming from other teams.
Currently it is kind of comfortable to stay where I am because so little is expected from me (and I can spend much of the day working on personal projects instead) but this job is absolute garbage for my professional development. While I live comfortably, I do have homeownership as an eventual goal and there's no way it's going to happen off of my current income.

And how do you identify if they’re good while you’re interviewing?
I’m approaching burn out at my current job and I think part of it may be due to my manager.
He usually treats our 1:1s as standups, and when I try requesting career advice or ask how I can improve, he gives unhelpful answers and quickly changes the subject. He also has a very fixed vision of how things should look and function and will sometimes redesign my work instead of giving me guidance.
He also doesn’t seem to understand (despite being told) that I have very limited engineering resources and that I need to compromise with my PM on business needs, so I’m often getting public messages asking why certain things aren’t fixed yet.
I can also tell he’s biased against the product I work on, so anything I share gets a lot of criticism. While I’d love to completely redo it or move to another team, neither are possible right now.
It’s making me incredibly stressed and I’m now doubting my own skills as a designer.

I am somewhat a new grad May 2022 working as an analyst at a big4 with no prior experience. My role has minimal client interaction. One bi-weekly and weekly meeting at the middle of the week that consumes between 1.5-3 hours of meetings per week. This meeting is a formality to prioritize bugs and organize our discussions around these systematic issues and from there we release new updates every 3 months. I feel like I’m working on so much ambiguity with no one on my team being able to give me a clear answer. The last release was extremely rough resulting in a lot of service now incidents. My team is always super busy and it’s very to hard to get in contact with my senior lead.
I’m going to be alone tomorrow during the client meeting for my team and since the team has been super busy with the new integration and the service now incidents. The client is now getting frustrated that we aren’t making progress on the bug analysis for our weekly meetings. It feels like every meeting is the team asking for more time. The system is large I never received training for this position. I was slowly given daily maintenance tasks. From the discussions with the client they are getting frustrated they aren’t receiving any updates on the bugs and I don’t know how to respond to their concerns since the system is vast and I haven’t learned enough to discuss. My manager has also stopped showing up to these meeting (busy with release and integration). I’m not so sure how to communicate to the client we are working very hard and we need more time. Normally my senior lead holds our hands (team of 4) but he will be on PTO along side another consultant. Leaving a new team member (4 months) and myself (1 year) to represent our team. I have a strong feeling the client will be upset that we will be asking for more time for 2-3 weeks in a row but my team has no other option. How do I respond professionally?

Well, this is a throwaway account because I don't want to be branded a you know what for no reason. Name changed for obvious reason. I'll refer to myself as J (26 M) and her as L (17 F). EDIT: While all this is happening, I should add that I am suffering from Major Depressive Disorder from years of bullying from elementary school up until my senior year in college...
So basically I was looking for a part-time job and started at this restaurant. She started a few days after me but I had to train her on the job. We became really good friends and I honestly thought of her as my little sister in a way, as my real little sister had basically cut ties with me and she has her own life. Anyway, fast forward and my girlfriend decides to dump me for no reason. So, in an attempt to kill myself, I drove my sports car really fast but a cop stopped me and took me to a kind of suicide watch place, where they bring people that attempt to harm themselves or have an active disorder. Anyways, I just basically had to call work saying that I was unable to come in and that's about it, so no one knew where I was, not even my housemates or my family, as they took away everything else and only allowed phone calls from their public line at certain times. The evening of the day I made that phone call, L somehow found me and brought our mutual good friend ( 19 M), we shall call him D. Both of them appeared at the foot of my bed when I woke up from a nap, and honestly, I thought that was the nicest thing someone had done for me in a long time. Anyways, after I got out a few days later and went back to work, I started to notice L had changed. She would get really flirty and constantly send me very, errr, you know..., photos that I obviously ignore and dodge by changing the subject like "hey your necklace looks good" or "did you know our other friend's car broke down" or some shit. Anyways, this went on for months, there was one time she even told another co-worker that me and her had this special corner and honestly, I was goddamn worried, because one, I am a full grown adult and two, she was like my little sister to me. But honestly, it only escalated from there. I had to actively dodge situations where it would just be me and her alone as she would start to do things like lay her chest on my hands or start playfully tickling me. I still dodged any romantic or sexual connection as I felt that we were like brother and sister. Suddenly one day, instead of being flirty or going back to the normal big bro/lil sis relationship we had before, she suddenly got really cold. Like, extremely I HATE YOU I WANNA SEE YOUR GUTS SPLAYED OUT ON THE FLOOR kind of hate. She backstabbed me to our boss saying that I was bossing people around while being lazy (I was promoted to a part-time management position). She would purposefully ignore me when I am making the schedule but answer anyone else, even the lazier manager. One time she left her jacket at work, so I took a picture of it and sent it to her and said "yo, your jacket here". She proceeds to ignore me and when I asked her did you take your jacket? She said our other friend told her about it, which was me to our other friend as I told him for some reason she was ignoring me. Anyways, many trips to the therapist and counsellor later, one of them told me to just tell her "I like you" and see, so I can either lecture her or if she doesn't like me anyway, she'll just leave, and honestly, I felt that I had to lose this relationship at that point, as she was getting really crazy. Anyways, I did tell her but instead of getting turned down normally or telling me she accepts, she proceeded to clock out immediately and never talk to me. She also told me all the flirtation she did of her sending photos was to everyone on her list, but she would purposefully send me angry faces or thumbs down when I ignore her, errr, photos... She unfriended me on the social media she used a lot, but for some reason kept me on the one I used a lot. She started posting stories to her public saying "I am so sorry, I fked up" with some sad song. Then she started posting how she wasn't happy with her hobbies anymore, and basically indirectly telling me like she's sorry, but since it's public and she said so herself, I sent it to "everyone", I kinda felt like I should ignore it but my dumbass thought I could rekindle our bro/sis relationship. So I tried to talk to her, with a peace offering, a really tasty drink that all of us liked. Anyways, she proceeded to tell me to stay the fk away from her. and so I respected her decision and did. But then, she unfollowed me but since I had not unfollowed her yet at that point, she posted a story about how she was stupid or some shit. Anyways. after I unfollowed her, our other friend told me that she stopped being active on the social media that I still use to this day. I don't know how to feel right now. I have quit my job as I don't want people to spread rumors about me, especially since I would've never done anything. But now that I am unemployed and school's out for the summer (I am still a college student...), I feel so dead everyday inside, like I am stuck and don't know what to do. Anyways, I am wondering what anyone else would do if they were in my position?
TL;DR: 17 F Coworker was hitting on me (26 M) for months, and after a period of dodging her, she started being very malicious. Told her I like her just to see what the result would be and ended up her doing some weird shit on social media that is basically indirectly telling me she wants to me to talk to her, while telling me to stay the fk away. She started spreading rumors so I had to quit the job too.

Hi all, I have been seeing my friends who barely say hi any more seemingly go past me in life in the past two years. They are getting married and having children while I’ve kept the same job and tried to pull myself together from unfortunate events.
My roommate had talked to his management about me and they were interested. I had an interview I thought was awful, I was crying after from how awkward it was and how little I could sell myself during it. Later I got a message saying one of the top management wants to discuss salary with me.
I will be making more than my father had been making, I will be able to pay off my car and school loans next year if I am wise, i will be basically doubling my salary and I will be able to send my family money so they can have more leeway!
I am so excited and scared. I can’t believe this is happening to me, i feel like I don’t deserve it. I will be telling my boss tomorrow. My heart’s pounding a mile a minute, I have no idea what to do with myself. Can anyone share how to go about what to do after getting a new job? Has anyone else gone through this huge change before?
